Covenant provides social work services to enhance your mental health and well-being and help you address challenges and concerns.
We assess your needs and strengths and help you work toward your goals of care.
Services
Services may include:
- supportive counselling
- psychosocial, cognitive and mental health assessments
- help conducting resident and/or caregiver support groups
- help addressing patient/resident/family/staff concerns
- information and education (e.g., making good decisions, caregiver stress, career or work options, end-of-life care, childcare options)
- help planning end-of-life care
- referrals to community resources and services (e.g., financial help, accommodation, transportation)
- liaison with outside agencies to offer additional support or boost what already exists
- help solving conflicts within families and with the healthcare system
- mediation between facilities and patients, residents or families
- communication with government agencies and departments on patients’ or residents’ behalf
- support with the transition home from hospital or to another care setting
- help with life transitions, grief, loss and other challenges
- specialized care for people living with dementia
- support with legal issues such as appointing powers of attorney and substitute decision-makers
